Information Needed depending on Visa Type

• Visitor's Data Sheet - All visitors to Georgia Institute of Technology, School of Mathematics expecting payment of any type.
• All VISA types must submit completed and signed W-8BEN http://www.irs.ustreas.gov/pub/irs-pdf/fw8ben.pdf . This form certifies residency status and is necessary for reimbursable expenses and the travel origin and destination point is outside the United States. Original receipts must be attached for processing. Once we receive the form and original receipts, a Georgia Tech Check Request Form will be completed and submitted to Accounts Payable for processing. A check will be mailed to the individual.
• If point of origin or destination is within the United States, all VISA types must provide a copy of I-94 and passport that includes visa classification. Visitors in the United States must have a visa classification that will permit expense reimbursement.
  • For F-1 visa, provide copy of I-20.
  • For J-1 visa, provide copy of IAP-66.
  • For B-1, B-2, WB, WT visas (visa waiver), the Certification of Academic Activity must also be completed to ensure visitor's stay at GT does not exceed 9 days.
• Original receipts for airfare and other related travel expenses. Receipts for meals (including tips) are not required, as per GT Travel Reimbursement policy.
• Social Security Number or ITIN is not required.
